Are there any forms or paperwork that I can complete and save some time before my first session?

Yes.  All my forms are PDFs so you'll need to have Adobe Reader on your computer.  If you don't, follow the link above and download it for free.  Print the forms that apply to you and bring them in for your first session:

Read and fill out both the Initial Interview Form and the Consent Form. Please have both parents and children 12 and over sign the consent form.

This consent form is for any boy joining The Guys' Group. Signatures are needed from both parents/guardians and children ages 12 and over.

Mail this form back to me to secure your seat at The Parenting Workshhop on Friday, March 15th at 5:00pm.
