Are there any forms or paperwork that I can complete and save some time before my first session?
Yes.  All my forms are PDFs so you'll need to have Adobe Reader on your computer.  If you don't, follow the link above and download it for free.  Print the forms that apply to you and bring them in for your first session:
  • Initial Interview Form - Please print this form and fill it out completely and bring to your first session.  
     
  • Child Informed Consent - If your child is entering treatment, please print this form and have each parent parent or guardian with legal custody sign.
     
  • Adult Informed Consent - This is for individual adults and adult couples entering treatment.  Please print this form and and sign it for your first session.
     
  • Guys' Group Consent Form - If you have a boy in Middle School who is joining the
    Guys' Group, this is the form for you.
